2. Personalized Nurse Support:  One of the standout features of our Chronic Care Program is the monthly phone calls from a dedicated personal nurse. Each patient is assigned a private nurse who becomes their primary point of contact for all health-related concerns. These regular check-ins are designed to monitor health status, manage symptoms, and provide education and support, ensuring that our patients never feel alone in managing their health.Regular Monitoring & Check-ins: Tracking key health indicators, such as blood pressure, blood sugar levels, and medication adherence, to identify trends and intervene early if necessary.
